 Special Events

As per the ODHA Special Events Guidelines, you must complete a Special Event Request Form

when you hold a team event that is not on SMHA purchased ice if you want the Hockey Canada insurance program to cover that event.  Such form must be submitted to the ODHA 72 hours prior to the event.

 

Events which qualify for Hockey Canada insurance coverage include dryland training and fundraisers.  Not all activities are eligible for coverage however.  See the this list

for additional information regarding possible coverage eligibility of specific events.

 

 

 

Here are some key points that should be part of the attached Special Event Request Form:

 

  1. No abbreviations (please have teams write their complete name).
  2. If an activity is taking place at a school, please indicate the name of the school as well as the school board.
  3. If it is a dryland training activity, please have team provide a detailed list of the dryland activities taking place.
  4. Name and address of the location of the event is a must.
