2010-11 NEW PLAYER Registration Information
Welcome to Stittsville Minor Hockey which offers both House and Competitive programs. Players who want to play either House or Competitive must first register with the Stittsville Minor Hockey Association.
Here are the age levels for SMHA players:
Juvenile: born in 90,91,92
Midget: born in 93,94,95
Bantam: born in 96, 97
Peewee: born in 98, 99
Atom: born in 00, 01
Novice: born in 02, 03
Initiation: born in 04, 05
There are two ways to register your player:
Download and complete all the items listed in the appopriate section of this page, then:
1. Drop off your paperwork at the registration evening which will take place on August 17th, from 6:30 to 8:30 PM at the Stittsville Legion on Main Street.
or
2. Mail in your application and other information to SMHA, PO Box 892, Stittsville, ON, K2S 1B1, attn Registrar.
Your membership in SMHA cannot be confirmed until all forms are completed correctly and payment is received. The recommended method is to drop off your forms at Registration Night so they can be checked for completeness and accuracy. If you mail your application we can't be responsible for mail delivery issues or incomplete forms.
Please note that making application through either method is NOT a guarantee that your child will be registered to play minor hockey in 2010/2011. Ice availability may restrict our ability to register all applicants.
You will only be contacted if your application has been rejected.
The following documents are required for both methods of registration.
For ALL new players:
1. a completed SMHA application form,
2. Signed and dated Code of Conduct Form
3. Cheque or Money Order for payment (cash is not accepted), see Fees page for amount.
4. Signed and dated NSF and Refund Policy Form
Please note that your application will NOT be processed until all documents are received and completed fully.
For first time hockey players (i.e. have never played in another association):
In addition to the items listed above for ALL players you will also need:
1. proof of residency (such as a copy of your Natural Gas invoice or City of Ottawa Water invoice showing your current address with the dollar amount blacked out). Your home should be in the area marked off on this map.
2. proof of age (photocopy of birth certificate, passport etc)
Please note that your application will NOT be processed until all documents are received and completed fully.
For hockey players who have moved to Stittsville but who have played with another association within District 4 (any of Almonte Pakenham, Carleton Place, Osgoode Rideau, Richmond Munster, Perth Lanark, or West Carleton) last season:
In addition to the items listed above for ALL players you will also need:
1. proof of residency (such as a copy of your Natural Gas invoice or City of Ottawa Water invoice showing your current address with the dollar amount blacked out)
2. proof of age (photocopy of birth certificate, passport etc)
3 . a completed ODHMA Declaration of Residence form (we will get all the signatures for you). This transfer document is an ODHA and Hockey Canada requirement and is required before the SMHA can register your child.
Please note that your application will NOT be processed until all documents are received and completed fully.
For hockey players new to SMHA but who have played with another association outside of District 4 (so any association other than Almonte Pakenham, Carleton Place, Osgoode Rideau, Richmond Munster, Perth Lanark, or West Carleton) last season:
In addition to the items listed above for ALL players you will also need:
1. proof of residency (such as a copy of your Natural Gas invoice or City of Ottawa Water invoice showing your current address with the dollar amount blacked out)
2. proof of age (photocopy of birth certificate, passport etc)
3. a completed and signed Declaration of Residence form. (we will get all the signatures for you)
4. If your previous association in Ontario, a completed Declaration of Good Standing signed by your previous association. This simply says that you were a member in good standing and that you don't owe them any money. If your previous association was outside Ontario, a letter stating from the previous association stating this is fine.
Please note that your application will NOT be processed until all documents are received and completed fully.
For hockey players who live outside of Stittsville but who wish to play in the SMHA:
In addition to the items listed above for ALL players you will also need:
1. A Transfer Form (we will get the signatures for you)
Please note that your application will NOT be processed until all documents are received and completed fully and the Transfer Form is signed by SMHA and District 4 Executives.
Payments
Your cheque, money order or bank draft should be dated September 1, 2010 and made payable to "Stittsville Minor Hockey Association".

WAIT! Register yourself! Without volunteers there would be no hockey in Stittsville so why not step up and offer your services - on the ice or behind the scenes.
