If your player withdraws from the SMHA hockey program, you may request a refund using this form.
- Before Sept 1, 2010: Full refund less a $50 Administration fee
- Sept 2 - Oct 15, 2010: Refund of 50% of the membership fee
- Oct 16 - Nov 30, 2010: No refunds except where a player receives a season-ending injury. With a written certification by a doctor, a 50% refund
- After Nov 30, 2009: No refunds for any reason
